011. This website is information, not medical care
The content here is general health information. It is not medical advice, it is not a diagnosis, and it is not a treatment recommendation. Using this site, filling in a form, booking an appointment, or exchanging messages with us does not create a provider-patient relationship. That relationship begins only when you are accepted as a patient and care actually begins.
Never disregard professional medical advice, or delay seeking it, because of something you read here.
022. Emergencies
If you are having a medical emergency — including chest pain, fainting or near-fainting with injury, difficulty breathing, or thoughts of harming yourself — stop and call 911 or go to the nearest emergency department. If you are in crisis, you can call or text 988. This website is not monitored around the clock, and nobody here will see an urgent message in time.
033. Screening and assessment tools
Some pages offer questionnaires and self-assessments. These are screening tools, not a diagnosis. A score does not tell you whether you have a condition, and no clinician reviews your answers in real time. Results are educational and are meant to help you decide whether to have a conversation with a clinician — ours or anyone else's. Treat the output as a starting point, never as an answer. How we handle what you enter is described in our Privacy Policy.
044. Booking and communications
Scheduling pages let you request or book a time. Booking a slot is a request; we confirm it separately, and we may need to reschedule. If you give us a phone number, we may contact you about your appointment by call or text — see our SMS Terms. Ordinary email and text are not secure channels; please do not use them for clinical detail.
055. Payment
MindForge does not bill insurance. Our approach to payment, superbills, and estimates is described on our payment pages, and you have the right to a Good Faith Estimate of expected charges — see Good Faith Estimate.
066. Intellectual property
The content, design, text, graphics, logos, and marks on this site belong to MindForge or its licensors and are protected by copyright and trademark law. You may view and print pages for your own personal, non-commercial use. You may not reproduce, republish, or use our content or marks commercially without our written permission.

Do not use this site to break the law; to interfere with its operation or security; to scrape, crawl, or harvest data by automated means without our permission; to submit false information or impersonate anyone; to upload malicious code; or to attempt to access areas you are not authorized to reach.

To the fullest extent the law allows, MindForge is not liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising from your use of this site, and our total liability for any claim relating to the site will not exceed one hundred dollars ($100) or the amount you paid us to use this website, whichever is greater.
Nothing in this section limits liability for the professional care we provide to our patients. These terms govern your use of a website; they have nothing to do with the standard of care owed to a MindForge patient, and we do not attempt to limit that.
