01Our policy
MindForge does not discriminate. We provide care and services without regard to race, color, national origin, ethnicity, religion, age, sex, sexual orientation, gender identity or expression, marital status, veteran or military status, disability, genetic information, or any other characteristic protected by applicable federal, state, or local law.
This applies to everything we do: who we accept as a patient, how we schedule, how we provide care, how we communicate, how we handle payment, and how we treat people who work here.
02Communication access
We provide, free of charge and on request: aids and services for people with disabilities — written material in large print, accessible electronic formats, and other formats, and reasonable accommodations for visits, including for people whose conditions make standard clinical environments difficult; and language assistance for people whose primary language is not English — interpretation and translated material, where we can reasonably provide it.

03If you believe you have been treated unfairly
Tell us. We would rather hear it and fix it. How to raise it:
- Tell any staff member. Most concerns are resolved on the spot by the person you are already talking to.
- If it is not resolved, ask for the Practice Manager, or contact them at (214) 730-6463 or info@mindforge.health. You can raise a concern verbally or in writing, and you may ask someone to do it on your behalf.
- What happens next: the Practice Manager is made aware of every concern within five days, and within 24 hours for anything urgent. Concerns involving clinical care are reviewed with the Medical Director; concerns involving nursing or clinical staff are reviewed with the Director of Nursing. We will tell you the outcome.
- We review concerns as a group, at least quarterly — not to assign blame, but to learn from them, improve care, improve safety, and spot patterns.
We will not retaliate against you for raising a concern.
