We do not collect health information on this form.

Before you book anything — and long before any traveler books a flight — send us what already exists. A clinician reviews your prior imaging, specialist notes, and test results and tells you what they see: what's been covered, what's missing and whether it matters, and whether MindForge is the right next step. It's a fixed-fee review — the cost is shared plainly before you send anything — entirely on your schedule, from wherever you are.

MindForge doesn't bill insurance — by design. Direct-pay is what makes an individualized, intensive program possible rather than care shaped by billing codes. HSA/FSA and financing options exist, and a written Good Faith Estimate precedes any commitment.

Why we work this way

You've likely done years of testing already; repeating it helps no one. And for the conditions we treat, a scheduled phone call can be a genuinely hostile ask — screens hurt, phones drain, symptoms don't keep office hours. Records review is the lane built for that reality: you send what exists, on your schedule, and we do the work — your history speaks for itself.

What to send

• Prior imaging reports (MRI/CT — the written reports matter more than the discs)
• Specialist notes — neurology, ENT, cardiology, concussion clinic, whoever you've seen
• Test results — vestibular testing, tilt-table, labs, neuropsych
• A few lines in your own words: what happened, when, what's been tried, what you want back

Missing pieces are fine. Telling you what's missing and whether it matters is part of the review.

How to send it securely

Send records the way that's easiest for you: call (214) 730-6463 and the care team will set up the secure hand-off — a HIPAA-appropriate channel, arranged on that call. Records go only to the clinical team, and nothing is shared without your authorization. Please don't send records over plain email.

What you'll hear back

Our care team follows up by the method you choose with the clinical review's answer: what we saw, what a Discovery Day would add — and whether it would — and a named route elsewhere if we're not the right place. A "not us, and here's who instead" is a real outcome here, and we treat it like one.
