MindForge provides instrumented neurological evaluation and structured, intensive outpatient rehabilitation for the patients who exhaust conventional pathways — post-concussion syndrome, POTS and dysautonomia, vestibular disorders, FND, and acquired brain injury among them. We function as an adjunct to your care, not a replacement for it: your patient remains your patient, and you receive findings and outcomes at every stage.
What your patient gets
A single comprehensive diagnostic day (the MindForge Discovery Day): quantified assessment battery — posturography, VNG-class oculomotor testing, autonomic measures including orthostatic protocols, cognitive and validated symptom instruments — plus clinical examination, same-day report of findings, and a costed, structured care plan. If findings don't support our involvement, we say so and refer onward; a clean rule-out with objective data is a useful result for you either way.
Treatment, where indicated, is delivered as concentrated multi-week intensives integrating neurorehabilitation, functional-medicine support, and clinical strength & conditioning under standardized clinical protocols individualized from the patient's assessment data — with objective re-testing against the patient's own baseline at defined intervals.
What you get back — every time
- Evaluation summary after the Discovery Day: findings, interpretation, plan — or the rule-out and our onward referral recommendation.
- Outcome reporting at program completion: pre/post objective measures and validated instrument scores.
- No black boxes: our assessment-driven clinical algorithms are documented, and we're glad to walk any referring physician through the methodology — the evidence basis is public.
Evidence basis, briefly
Modality anchors include Grade-A meta-analytic support for vestibular rehabilitation, RCT support for sub-symptom-threshold aerobic exercise post-concussion (Buffalo-protocol class), structured exercise rehabilitation for POTS in the Levine-protocol lineage (UT Southwestern provenance), and published multidisciplinary motor-retraining programs for FND. Program-level care is an evidence-informed synthesis, instrumented per the MRC complex-interventions framework — outcomes registry from day one.
Good referral candidates
Persistent post-concussive symptoms beyond expected recovery windows · suspected or confirmed POTS/orthostatic intolerance needing structured, supervised reconditioning · chronic dizziness/vertigo with unrevealing structural workup · FND with positive rule-in signs, motor phenotypes especially · post-TBI/stroke patients who've plateaued in, or exhausted, covered therapy. When in doubt: send records — the review costs your patient nothing.
Not appropriate for us
Acute emergencies, active structural disease requiring surgical or acute medical management, primary psychiatric crises, and patients needing medication management as the primary intervention. We route these back with documentation rather than enrolling them — and we'd rather decline a referral than dilute your trust.

The economics, plainly: MindForge is direct-pay; patients receive pricing plainly (a fixed evaluation fee shared before booking; program quotes in writing with a Good Faith Estimate). We pay nothing for referrals and accept nothing for them — your patient's trust in you is not for sale, and neither is ours.
